If you have eligible care needs, your local council may fund some or all of your care. A social worker or care manager will carry out an assessment of your needs and finances. If eligible, Direct2U can be instructed as your provider.

NHS Continuing Healthcare
Adults with complex, primarily health-related needs may qualify for fully funded care through NHS CHC, at no cost to the individual. Direct2U works with CCG and ICB commissioners to deliver NHS-funded packages.
- Adults with a 'primary health need' that is complex, unpredictable, or intensive may qualify
- A multidisciplinary team carries out a CHC assessment — your GP or care manager can refer you
- Fast-track CHC is available for those in the last stages of life, enabling rapid care to begin
NHS Continuing Healthcare is entirely free to the individual — it covers the full cost of care regardless of your personal finances.
