Working in Partnership with Your Palliative Care Team

Direct2U does not provide clinical palliative care — that is the role of your GP, district nurse, Macmillan nurse, and hospice team. Our role is to provide the daily care, comfort, and human presence that enables clinical care to be most effective.

A good palliative care arrangement at home involves many professionals working together — each contributing their expertise to the person’s comfort and dignity. District nurses manage clinical symptoms and administer medications. Macmillan nurses provide specialist palliative advice and emotional support. The GP coordinates the overall medical plan.

Direct2U provides the continuous, daily, human layer of care — the presence in the home between clinical visits. We ensure the person is never alone or uncomfortable, that medications are taken as prescribed, that personal care is delivered with the utmost gentleness, and that family members feel supported and not alone in carrying the weight of this time.

We attend multidisciplinary meetings where appropriate, follow the agreed care and advance care plan without exception, and communicate any changes in the person’s condition promptly to the clinical team.

We arrange a home assessment as quickly as possible — often the same day or the following morning. We meet with the individual, their family, and wherever possible their district nurse or GP, to understand the current situation and design the right care package.

If the person is eligible for NHS Continuing Healthcare under the Fast Track pathway, Direct2U can support and expedite this application. Fast Track CHC can fund palliative care rapidly at no cost to the individual.

Care Begins — Immediately if Needed

In cases of urgency, care can begin within hours of the initial contact. We assign a small, consistent team, follow the care plan precisely, and remain in close communication with the family and clinical team throughout.

The most important funding route for palliative care. Anyone with a rapidly deteriorating condition and limited life expectancy may qualify for NHS Continuing Healthcare under the Fast Track pathway — which can fund 24-hour palliative home care rapidly at no cost to the individual or their family. Those who do not qualify for NHS CHC may be entitled to a funded care package from the Local Authority following a social care needs assessment. Direct2U works with North London boroughs to deliver Local Authority-funded palliative packages.
